Terms of Service

Last updated: Oct. 21, 2026

These Terms of Service govern your access to and use of https://elevatecolorado.com and the products, programs and administrative services offered by Elevate Integrative Wellness (“Elevate,” “we,” “us” or “our”).

By accessing this website, creating an account or placing an order, you agree to these Terms, our Privacy Policy and our Return and Refund Policy. If you do not agree, do not use this website or purchase its services.

1. Not for Medical Emergencies

This website and its telehealth services are not intended for medical emergencies. Call 911 or seek immediate in-person medical care if you believe you are experiencing an emergency.

2. Eligibility

You must be at least 18 years old, legally capable of entering into an agreement and physically located in a state where the requested service is available at the time of your medical evaluation.

You agree to provide complete, current and accurate information about your identity, location, medical history, medications, allergies and health conditions.

3. Telehealth and Independent Medical Providers

Elevate facilitates access to telehealth evaluations performed by licensed healthcare providers through Qualiphy or another authorized provider network.

Clinical services are provided by the applicable licensed provider or medical group, not by this website’s ecommerce software. Medical evaluations, diagnoses, treatment recommendations and prescribing decisions are made solely by the licensed provider exercising independent professional judgment.

Use of the website or purchase of a program does not establish a provider-patient relationship with Elevate unless expressly stated. A provider-patient relationship may be established with the medical group or licensed provider conducting your evaluation.

Qualiphy and participating medical providers may require you to accept additional patient terms, telehealth consent forms and treatment-specific disclosures.

4. No Guarantee of Approval or Results

Purchasing a consultation, product or program does not guarantee medical approval, a prescription, medication fulfillment or a particular health outcome.

A provider may decline treatment, request laboratory testing or medical records, recommend an in-person examination or determine that another form of care is more appropriate.

Individual results vary. No statement on this website should be interpreted as a promise or guarantee of weight loss, symptom improvement, aesthetic results or other outcomes.

5. Prescription Products

Prescription products are available only when prescribed by an appropriately licensed healthcare provider after determining that treatment is medically appropriate.

You may not purchase prescription products for another person, share medication, alter prescribed dosing or use medication contrary to the provider’s and pharmacy’s instructions.

Product photographs and descriptions are illustrative. The prescribed formulation, strength, dosage, quantity and instructions are determined by the provider and dispensing pharmacy.

6. Compounded Medications

Some treatments may involve compounded medications when prescribed for an identified patient. Compounded drugs are not FDA-approved, and the FDA does not verify their safety, effectiveness or manufacturing quality before marketing in the same manner as FDA-approved drugs.

Your provider will determine whether a compounded medication is clinically appropriate. Availability, formulation and pricing may change according to provider judgment, pharmacy availability and applicable law.

7. Pharmacy Fulfillment and Shipping

Approved prescriptions may be transmitted to an independent licensed pharmacy. The pharmacy is responsible for preparing, labeling and dispensing medication in accordance with the prescription and applicable law.

Delivery dates are estimates. Provider review, pharmacy processing, weekends, holidays, product availability, weather and carrier delays may affect delivery.

You are responsible for providing an accurate shipping address and promptly retrieving delivered medication. Follow all storage instructions supplied by the pharmacy.

8. Prices and Payment

Prices are displayed before checkout and may include administrative services, a telehealth consultation, medication and shipping, depending on the selected program.

Services are generally offered on a cash-pay basis. Unless specifically stated otherwise, Elevate does not submit claims to Medicare, Medicaid or commercial insurance and does not guarantee reimbursement.

You authorize us and our payment processor to charge the payment method provided for all disclosed amounts, taxes and applicable fees.

9. Refunds and Cancellations

Refunds, retained consultation fees, cancellations, damaged shipments and prescription returns are governed by our separate Return and Refund Policy, which is incorporated into these Terms.

Prescription medication generally cannot be returned or exchanged after it has been prepared, dispensed or shipped. Telehealth consultation fees may become nonrefundable once an evaluation begins or is completed.

10. Recurring Programs

If you purchase a recurring program, you authorize the disclosed recurring charges until the program is canceled according to the cancellation terms presented during enrollment.

Cancellation prevents future eligible renewals but does not ordinarily refund services already performed or medication already processed. Submit cancellation requests at least [number] hours before renewal by contacting [support email].

Delete this section if you do not offer automatic renewals.

11. Website Information

Website content is provided for general educational purposes and is not a substitute for personalized medical advice, diagnosis or treatment.

Do not delay or disregard advice from a qualified healthcare professional because of information found on this website.

12. User Responsibilities

You agree not to provide false information, impersonate another person, misuse prescription services, interfere with website security, attempt unauthorized access or use the website for unlawful purposes.

We may suspend access, cancel an order or discontinue services when reasonably necessary to protect patients, providers, pharmacies or the integrity of the platform.

13. Privacy and Electronic Communications

Your information will be handled according to our Privacy Policy and applicable privacy notices. Information submitted through Qualiphy, a medical group, provider, pharmacy or payment processor may also be governed by that party’s privacy practices.

By providing your contact information, you consent to transactional communications regarding orders, evaluations, prescriptions and account activity. Marketing communications are subject to separate consent where required.

14. Third-Party Services

This website may connect to services operated by Qualiphy, medical groups, licensed providers, pharmacies, laboratories, carriers and payment processors. These parties may have separate terms and privacy policies.

Elevate does not control the independent clinical judgment of providers or the professional dispensing decisions of pharmacies.

15. Disclaimer of Warranties

To the extent permitted by law, the website and nonclinical administrative services are provided “as is” and “as available.” We do not warrant uninterrupted website access, error-free operation, medication availability or particular treatment results.

Nothing in these Terms limits rights or warranties that cannot legally be excluded.

16. Limitation of Liability

To the fullest extent permitted by law, Elevate will not be liable for indirect, incidental, special or consequential damages resulting from use of the website or administrative services.

Nothing in these Terms excludes liability that cannot be limited under applicable law or alters the professional responsibility of a healthcare provider or pharmacy for services that party provides.

17. Intellectual Property

Website text, branding, graphics and original content belong to Elevate or its licensors and may not be reproduced, distributed or commercially used without written permission.

18. Changes to These Terms

We may revise these Terms when our services, vendors or legal obligations change. Revised Terms become effective when posted with an updated effective date. Continued use of the website after an update constitutes acceptance of the revised Terms.

19. Governing Law

These Terms are governed by the laws of the State of [state], without regard to conflict-of-law principles. Any dispute will be handled in the courts located in [county and state], unless applicable law requires otherwise.
